It's an amazingly cool toy that allows you to control the in-game camera just by turning your head. A small infrared sensor sits on top of your monitor, and a small frame holding three reflective points attaches to either your headset or a normal ballcap.
I just bought mine a couple weeks ago, and now can't imagine playing any flight/space sim without it. The effect is like tripling the dimensions of my monitor so I can see in all directions all the time. The software takes a little tweaking to get the settings all comfortable for my personal preference, so when I'm looking straight ahead the camera sits still, and when I turn my head a few degrees to either side the camera pans 90 degrees.
There's also a free version of the software out there, called Freetrack. But with Freetrack you have to put the reflectors and IR camera together yourself, and there's some dispute over the legality of the software; TrackIR claims Freetrack pirated their code.
So TrackIR is expensive but works great. Freetrack is free and also works great, but is a more intensive DIY project and has allegations of some shady dealings.
